Position Summary

The Advanced Practice Provider delivers comprehensive patient assessment, diagnosis, treatment, and ongoing clinical management in collaboration with supervising or partnering physicians. This role provides high-quality, evidence-based care across diverse patient populations and clinical settings.

Key Responsibilities

Perform comprehensive patient assessments, including history, physical exams, and diagnostic interpretation.
Diagnose and manage acute and chronic conditions within scope of practice.
Develop and implement individualized treatment plans, including medication management and therapeutic interventions.
Provide patient education, counseling, and follow-up care to support long-term health outcomes.
Collaborate with physicians, nurses, and multidisciplinary teams to coordinate patient care.
Document all clinical encounters and treatment plans in accordance with regulatory and organizational standards.
Participate in quality improvement initiatives, care coordination meetings, and clinical reviews.
Maintain current knowledge of evidence-based practices, clinical guidelines, and regulatory requirements.
Ensure compliance with HIPAA, patient safety standards, and state/federal regulations.

Qualifications

Master’s degree or higher from an accredited NP or PA program.
Active and unrestricted state license as a Nurse Practitioner or Physician Assistant.
National certification (AANP/ANCC for NP; NCCPA for PA).
DEA registration with appropriate schedules.
Strong clinical assessment, diagnostic, and communication skills.
Experience in primary care, specialty care, or acute care preferred (depending on setting).

Experience managing diverse patient populations across various clinical environments.
Familiarity with EMR systems and documentation best practices.
Commitment to patient-centered, collaborative care.
Ability to work effectively within multidisciplinary teams.

Work Environment

May include outpatient clinics, inpatient units, specialty practices, urgent care, or integrated care settings.
Schedule and call requirements vary by organization.

Skills
