Position Summary
The Advanced Practice Provider delivers comprehensive patient assessment, diagnosis, treatment, and ongoing clinical management in collaboration with supervising or partnering physicians. This role provides high-quality, evidence-based care across diverse patient populations and clinical settings.
Key Responsibilities
- Perform comprehensive patient assessments, including history, physical exams, and diagnostic interpretation.
- Diagnose and manage acute and chronic conditions within scope of practice.
- Develop and implement individualized treatment plans, including medication management and therapeutic interventions.
- Provide patient education, counseling, and follow-up care to support long-term health outcomes.
- Collaborate with physicians, nurses, and multidisciplinary teams to coordinate patient care.
- Document all clinical encounters and treatment plans in accordance with regulatory and organizational standards.
- Participate in quality improvement initiatives, care coordination meetings, and clinical reviews.
- Maintain current knowledge of evidence-based practices, clinical guidelines, and regulatory requirements.
- Ensure compliance with HIPAA, patient safety standards, and state/federal regulations.
Qualifications
- Master’s degree or higher from an accredited NP or PA program.
- Active and unrestricted state license as a Nurse Practitioner or Physician Assistant.
- National certification (AANP/ANCC for NP; NCCPA for PA).
- DEA registration with appropriate schedules.
- Strong clinical assessment, diagnostic, and communication skills.
- Experience in primary care, specialty care, or acute care preferred (depending on setting).
Preferred Skills & Experience
- Experience managing diverse patient populations across various clinical environments.
- Familiarity with EMR systems and documentation best practices.
- Commitment to patient-centered, collaborative care.
- Ability to work effectively within multidisciplinary teams.
Work Environment
- May include outpatient clinics, inpatient units, specialty practices, urgent care, or integrated care settings.
- Schedule and call requirements vary by organization.
Skills
